The Skyview Tower (1969-1969)
The Sky View Tower was a 494ft tall[1] skyscraper with its restaurant The Sky View that opened and closed on May 16, 1969. Although extremely ritzy and coveted, the tower saw a rushed construction, being opened five months ahead of schedule with poor operational standards and abysmal structural quality.
Though these flaws could've led to a total collapse and the deaths of all guests during its opening day, such a disaster was entirely prevented by Iris Campbell warning patrons of the impending danger. The tower was closed for inspection and renovations, but never reopened before being demolished.
Theatrically, The Sky View collapse is the sixth major disaster in the series, as seen in Final Destination Bloodlines, and the second disaster chronologically given the Mornington Crescent.
The Sky View is the chosen venue for Paul Campbell to bring his girlfriend Iris on a date, all with the intentions of proposing to her. Throughout the night, Iris begins to spot signs of the tower's poor construction and operating standards. A bartender opens a bottle, launching the cork into a chandelier above and breaking one of the glass shards which drops with enough force to crack the reinforced glass dance floor beneath the guests. The crack slowly spreads as Iris is taken to the viewing deck for Paul to make his proposal. While up there, the high winds blows Daphne Wo's hat from her head while Alfred Milano continuously throws coins off the side.
Premonition
After being warned to stop by a guard, Alfred throws another penny off the balcony just as a strong gust catches it and blows it into an air vent. The penny then gets stuck in the blades of a fan, which eventually breaks, launching one of the fan's blades into a gas pipe, puncturing it. Flammable gas then slowly builds up and makes its way through the ventilation system.
After accepting Paul's proposal and revealing her pregnancy to him, Iris returns to the dance floor to celebrate their engagement with the live band. As people keep dancing, the glass floor begins to crack while the rivets holding the tower's supporting beams together begin to pop off from the weight. As the music reaches a crescendo, the floor suddenly shatters completely, sending those on it plummeting to the ground below. The falling dancers cause one of the tower's support beams to break while Paul is left clinging to the sharp fragments of glass left behind. Despite Iris' attempts to reach him, he loses his grip and hits his head on the broken support, falling to his death.
The accident causes a mass panic as the guests try to evacuate. Amidst the chaos, Beatrice Fuller runs into a chef carrying a still flaming pan, causing her to get set on fire. She then runs into the accumulated gas that has been leaking through the vents, creating a large fireball that incinerates many guests, including JB's mother Evie. As one half of the restaurant burns, Chet persuades survivors to evacuate through the stairs, but the stairwell suddenly crumbles and causes more people to fall out of the building, except for Alfred, who escapes. Chet then tries to get people to escape using the elevator, but it is quickly overloaded with people. The mechanism snaps from the weight and sends the elevator falling with everyone inside being killed from it crashing to the ground, Crushing the maître d' between the doors and bisecting him vertically in the process. This leaves the remaining people trapped on the restaurant level.
With the structural integrity compromised, a large portion of the restaurant begins tilting downward, causing everyone except Iris to slide into the outer glass windows. The penny that caused the gas leak lands on the band's grand piano, which is balancing precariously on broken floorboards. The extra weight added by the penny causes the piano to begin sliding towards the guests, crushing Daphne and causing the windows to shatter and send the others plummeting to their deaths. Alfred, who has just reached the ground, is crushed by the falling piano.

She notices JB and moves to save him as the other half of the restaurant begins to tip over from the fire. Iris manages to save JB and escape the collapse, but only catches herself by her engagement ring caught between two pieces of rebar.
