The PHP generator now creates variables for the NotNull accessors.

Fixed the PHP code generator to properly use the default prepared statement name.

The PHP generator prepared statements now uses the default prepared statement, rather than a generic named statement.

Loading a design document now changes the titlebar contents.

The main window title bar now shows the design document filename first, then the application name. This makes it easier to find the proper document in the title bar when multiple instances of the program are running.
