Survey Builder was born out of the need to convert surveys into a web-based format that meets university ethics approval. The university already has a good survey system called "Web Survey" that allows you to build a survey with building blocks. You can add questions, and select the type of input you expect for those questions (e.g. select list, multiple choice answer, etc..). This system works great for simple surveys that whose design fits into its rigid structure.
