I don't have this, but on the same theme,

I have a fold out card in my wallet. At the top, ticking out and visible, it says Medical Info in big red letters.

then it's got Name, age, height, weight, blood type, NKDA, the basic assessment info. A note that I have sensory issues and might need a blanket and a dark quiet room to reorient after an Accident.

Then I have a List. Every single Rx and supplement I am on, how much, when I take it. All of them. It's a lot. Nurses LOVE THIS.

then there's a short summary of what my conditions are, what that means, and what it means relevant to emergency treatment. Like how I *need* rest and sleep and checking on me every couple hours during the night will reduce me to meltdown. Or how stress will rapidly worsen symptoms. various other notes.

Then there's a section of Medical History, why I cannot possibly be pregnant, surgeries I have had, still have an appendix, etc.

Then the final page has phone numbers, my emergency contacts, my insurance because the USA healthcare system is a hellscape, my primary care doctor, and chaplain preferences when in a hospital.

Every Doctor I have who has seen this says I oughta market the format, but it's literally just index cards trimmed to the size of a credit card so that it fits in the pockets in my wallet/phone case thing. Anyone with access to paper could make or get help to make something like this.Then I laminated it with clear packing tape so they all hold together and can fold.

I LOVE being at an appointment and being able to hand over everything they want to know on a neatly written card. There's no way I could list all my meds in a quiet appointment let alone an emergency situation.

Update:

I did end up in the ER this last year, trying to breathe with lungs rapidly filling with fluid

and I DID USE THAT CARD and it was vitally helpful and I am now recovering from a situation that could have gone VERY BADLY if we'd hit any delays in care or hadn't had that info right on hand.

I could barely get a word out and they kept asking me so many questions. The family members there with me kept trying to answer nurse questions about my illness and medications, getting them wrong, and I kept gasping, "CARD!" to get them the accurate answers.

If you have any medical conditions whatsoever- Make a Wallet Card. Don't wait until you need it.
