I know we like to joke about zuko doing 1 good act and fainting and falling sick but I wanna actually talk about it. Zuko is shown to do good acts throughout the show even as an antagonist. You could even argue his entire arc began when he did his good act of defending his people from being used as bait. The reason he falls ill after letting appa go is because for the first time, he has to let go of his purpose. The purpose that has been fueling him for like 4 years! He's worked endlessly and tirelessly to capture the avatar and he gives it up and he's lost now. In one decision he gave up his purpose, his country, his family, and his honour! That would make anyone have a breakdown!
It's also important to note that he doesn't make the choice fully on his own, he's pushed (for the first time) by iroh. So when you put all that together, you realize why he betrayed iroh and team avatar when he got the option of regaining all of that!
The horror of getting what you want and it not being what you thought, of the many instances of violence he saw his country commit and he himself commit, it all comes to a head when he's faced with the real history of who he is and the lies his family and country have been telling. In this instance he gets a new purpose, and he chooses, by himself, to rebel and do good and this time he doesn't have a mental breakdown
