I was graciously given four extremely awesome outfits for Eren as seen in its official reference page: https://www.furaffinity.net/view/39969782/
I'll be drawing the remainder of those outfits! This one was a particularly fun start.
Eren Mautam represents an unchosen path... that of an XY chromosomal person who started transitioning to become female-gendered, but desisted.
Eren has a dim view of gender identity. For example, "it" is its chosen pronoun, and it delights in how clumsy that makes conversations.
Though Eren has underwent some amount of modification, it has ultimately decided to salvage its life by optimizing the body it's fated to have. Part of this includes weight training, as implied by that beefy arm.

It is important for me to make Eren and Serenity very distinct. Truly, they both find happiness at the end of their individual paths, though they diverge greatly.
Eren's path is somehow more torturous, but "a smooth sea does not make a skilled mariner".

The syringe aspect of the tattoo was sort of an accident. ROTDA did the outfit you see in the upper right of the picture, and the lines underneath just happened to be such that I saw them as hypodermic needles (Rorschach test anyone?) so I went with it. It makes so much SENSE, in a few different related contexts.
I don't think I've ever drawn a proper mouse. I did a horrifying fanart of Gadget a few years ago that I'd prefer to not unearth.

Good question, thanks for asking it. It allows me to reveal a little more of Eren's character.
Short answer: No, it does not use real-world human drugs (I'm presuming you're not referring to caffeine: https://www.furaffinity.net/view/23554141/).
Medium answer: Since my characters are all ambassadors of ME, and I'm straightedge, they don't use human drugs. I use them to tell my story, and I have no experience with drugs, so it wouldn't be sincere storytelling to include that element.
Long answer: The above two answers plus these other elements: the syringes are more than an aesthetic thing. Eren works for Pestilence. As such, a syringe is one of the weapons in its arsenal to spread disease. This can be done actively, as in melee combat, or passively like setting biohazard traps for folks to step on/walk into (or maybe just set boxes of the things adrift at sea to pollute some beach somewhere XD).
All of my characters are more metaphor than literal. It's what allows them to do things like murder and torture other creatures without the typical human world reprisals. In my headcanon, Eren eats toxic garbage, and if it were to inject anything, it would be nerve agent or hydrazine or something acutely horrible and deadly. Eren doesn't seek pleasure or pain relief. Eren seeks pain, and pleasure relief.
Sometimes folks give me gift art that includes my characters doing non-canon things, like smoking. I never turn up my nose at gift art, and I reconcile the disparity in my mind this way: people see us differently than we see ourselves, and if people assign activities to us based on their perception of us, that's a learning moment about how our energies are received by others. Message sent doesn't always equal message received.

Ohhhh I think I see what you mean. An anti-nurse... haha. Kinda makes me wanna draw Eren in a gothic (masculine) nurse's outfit (featuring green/black aesthetic) with syringes filled with some kind of dingy green substances, to emulate what it is you just said in a kind of comical understanding of this. When I think of syringes, I usually think nurse/healers, but it's also fun to think the opposite, now that you put the image in my head.
