Image / Media conflict and button missing

Summary

Image icon missing from toolbar, and when clicking "Insert > Media," it adds a non-playable embedded video instead of an image.

Step to reproduce

1. Edit content.
2. Click insert image, and select image.

Observed behavior

Inserts video embed in place of image.

Expected behavior

It should display an image and allow for image attributes to be edited.

MODX 2.x - 3.x

It sounds like you have the media plugin. Insert Image and Insert Media are two different things. Media is video/iframes/etc.

Can you provide what you have for your plugins and toolbar settings?

Insert Image isn't available. I can't see it in the menu dropdowns or toolbar.

I have the default settings upon installing.
