AFFIDAVIT RE: ONE & SAME PERSON


I, JAIME NESUS LORENZO, JR., of legal age, Filipino,
married, and a resident of 71 MH del Pilar St., Tacloban City, after
having been first sworn according to law, hereby depose and state:
THAT--1. I was born to parents JAIME G. LORENZO, SR. and
FRANCISCA J. NESUS on 20 APRIL 1961.
2. I am presently a Brgy. Kagawad of Brgy. 15, Tacloban City,
and popularly known in the community by my alias JAMES
LORENZO.
3. That is why when my friend, SUSAN BONCAN and
JOSEMARIE RUSIANA in the USA sent me financial assistance
through the Moneygram, they addressed me (the receiver) in my alias
as JAMES LORENZO.
4. I am executing this affidavit to vouch that JAMES LORENZO
is one and the same person as herein affiant JAIME N. LORENZO,
JR.
IN TRUTH WHEREOF, I have hereunto affixed my signature
this 9 May 2015 at Tacloban City, Philippines.
