Transcript Request for Students

This transcript request form is for undergraduate students at the University of Massachusetts Amherst to request official or unofficial copies of their academic transcripts. It requests the student's name, ID number, date of birth, contact information, dates of attendance, and the number of transcripts needed. The student can choose to have the transcripts mailed to themselves or another recipient, picked up in person, or held until certain grades or a degree are recorded. There is no fee for requests from students who enrolled after September 1990.

Charges:
There is no fee for students who have entered the University as of September 1990 or later.
If you entered the University prior to September of 1990, there is a service fee of $6.00 for each transcript.
