hazard.
5 Existing control measures Identify the kinds of control measures already in place
to contain the above said hazard. ( Technical/
Administrative Controls)
6 Risk rating Use the risk rating calculator and find the current risk
level which should be written as Likelihood X
Consequences =Risk ( LXC=R)
7 Additional Controls Recommend a further technical (engineering) control
to contain the hazard
8 New risk rating ( Residual) Calculate the residual risk which may persist in the
workplace yet after employing the technical control
measure. This should be lower than 6
9 Action/ monitored by whom? Write the designation of the personnel who monitors
the activities on site specific to the hazard which has
been found.
10 Action/ monitored by when? Write the specific date which may be the target to
employ the technical measures recommended. This
shall be calculated from the date of risk assessment
11 Review Date This shall be written by calculating, the maximum
timescale given for employing the technical controls
recommended, compared with the date of risk
assessment. The course providers name must be
