Fix a few issues in markdown help files

Description

• Add missing entries under INPUTS header in markdown files
  • I believe this will fix issue Get-AzureRmStorageAccount missing output documentation #4903
• Fix error in the header of PowerBI markdown help files
• Add missing help URL in the header of markdown help files
• Add check in CI to validate that all markdown files have a corresponding help URL in their header

Checklist
