FIX: Remove mention of "REFACTOR" type

FEAT: Add guide and quick reference image

Create initial commit of README.md (the guide) and the quick reference image
(essentially a continuation of the initial commit - since I created the
repository on GitHub then these files locally, it is a 2-part initial commit.)
