Yeah, i found them.
They are left and right Shifts.

Enter or (both?) shifts to launch, left and right shift for flippers. Space bar to tilt.

no problem. By the way, both alts are to tilt sideways, though I found only good use for the normal tilt with space bar.

You can also use "Alt" and "Alt Gr" to left and right tilt!

Quick Exit => Left Shift + Esc
Contrast Adjustment => C
LED Display Toggle => L
Select View => 1-6

what are the buttons to put your name in for the high score?

Sorry i haven't played it for years but just try typing you're name or maybe hit enter or space before.

i know this is an old thread but for anyone who is looking for it, you can change the controls to your liking in the game install folder. there is a config setup exe there by the name "pincfg".

By default, with the 2 shifts you go forward and backwards with the letters / numbers
