It’s no wonder Dean thinks his entire worth as a being is defined by how many people he can save when everyone keeps reinforcing that belief.
From John all the way up to God and Death - It’s always about work to do, people to save, apocalypses to avert - never about Dean.
So of course he spirals down as soon as he fails to save people, especially the ones closest to him, because to him it means he’s failed on a fundamental level; that he, as a person, is a failure.

This actually makes me a little sad for Dean. Dean is not at all known for talking about feelings, his or anyone elses, and when he does have some moment of weakness, some verbal freak out, or some expression self doubt, the responses aren’t comforting, or reassuring. They’re not gentle, or kind. They’re rude and condescending, they toss around names, and instead of actively listening they discount his feelings and worries.
