I imagine that he was stealth/socially transitioned before hydra and then hydra sort of forced a medical transition after kidnapping him (not in a trans supportive way obv just realizing the soldier they grabbed was not a man like they thought and wanting to turn him into the male soldier they thought he was)
I think after hydra bucky also would not know how to handle his transition because while it was his decision his agency in his medical transition was another thing taken away from him by hydra and these alterations to his body that should have been liberating for him are now in an entirely different light and he has to go through the process of understanding his identity outside of the winter soldier in a lot of different ways at once
