APPLICATION FOR PROBATION

ACCUSED-APPLICANT, through the undersigned Public Attorney, to this


Honorable Court, respectfully applies for probation under the provisions of
Presidential Decree No. 968, as amended and declares that:

1. Accused pleaded guilty to the above-entitled cases and was imposed a


PRISON TERM NOT EXCEEDING SIX (6) YEARS and FINE of TEN
THOUSAND PESOS, BOTH of which he is including in this application.

2. He has not been previously convicted by final judgment of an offense


punished by imprisonment of more than 6 months nor a fine exceeding Php
1,000.00. The crime of which he was convicted is not an offense against the
security of the state or public order. He has not been once on probation under
the provisions of Presidential Decree No. 968, as amended, hence qualified to
avail probation law.

3. This motion is not intended for delay but for these premises alone.

WHEREFORE, premises considered, it is respectfully prayed that this


application for probation be given due course.
