APPLICATION FOR PROBATION
COMES NOW, Accused XXXX, assisted by the undersigned
counsel from the XXXXX, to this Honorable Court, most respectfully
applies for Probation under P.D. No. 968, as amended and avers that:
1. During the Promulgation of Judgment on 18 November 2021, Ac-
cused was sentenced the penalty of imprisonment one (1) year,
subject to subsidiary imprisonment, and ten thousand
(P10,000.00) Pesos fine, after accused availed of Plea Bargaining;
2. The offense for which accused was convicted is not a crime
against the Security of the State and Public Order;
3. Accused does not suffer from any of the disqualifications of the
Probation Law and he would like to avail the benefits and privi-
leges accorded by law to first time offenders;
4. Accused has not been previously placed on probation under the
provisions of P.D. No. 968, as amended; and
5. Accused is sincerely remorseful of having committed the offense
and if granted probation, he is willing to undergo supervision under
such terms and conditions that may be imposed by the Honorable
Court.

PRAYER
WHEREFORE, foregoing premises considered, it is most
respectfully prayed of this Honorable Court to give due course to this
Application for Probation and place herein accused-applicant on
probation in accordance with law.
Other reliefs, just and equitable under the premises, are also
prayed for.
