I know thats usually called Mastectomy but do you think doctors can just remove the nipple anyway?

For the double incision bilateral mastectomy, which is probably the most common type of top surgery, they have to remove and replace the nipple. You can just ask them to not put it back. That's what I did and it turned out fine:

My scars are a bit higher up than "normal" for a double incision; they usually try to put them along the underside of the pec muscle so they don't stand out so much. That's not related to the no-nipple thing though, I just had a weird surgeon.

But if you have very small breasts they might very well recommend a keyhole top surgery, which doesn't normaly include the removal of the nipple. So in that case your scars will probably end up where mine did if you ask for no nipple, since your skin might not be enough to stretch to under the pec muscle.
